We all dread the process of trying to squeeze a complicated and detailed medical history into a cluttered medical form.
Rather than feeling pressed for time and having trouble remembering all the little details of your medical history, you can complete this paperwork at your leisure — in the comfort of your own home or office. All you need is a computer, an internet connection and a PDF viewer. (Most of you automatically have this. If not, you can easily download one for free.)

I encourage you to take your time with it and be as thorough as possible.
When finished you can save a copy for your records.
E-mail the completed history form back to me — preferably within 12 to 24 hours of our appointment — and I will review it carefully before our meeting.
A few things about privacy and security
Your privacy is extremely important to me. I never, ever sell or share your information. I always keep patient security and privacy top of mind. Everything is locked down tight.
+ There is encrypted security on this form so it should be safe going through e-mail — as long as you don’t type sensitive information in the body of the e-mail itself.
+ If you are still concerned about sensitive information like social security number or really private medical history details, skip that part, and we’ll add it in once you are in my office. Easy.
+ If you just don’t feel comfortable at all sending this form via e-mail for whatever reason, that’s okay. I have a back-up plan — schedule a free 15-minute conversation with me and we’ll chat over the phone or Skype and I’ll fill your information in that way OR just wait until your first appointment, come about 15 minutes early and we’ll do it the old-fashioned way. No problem!
